WHY THIS SUMMIT MATTERS

Innovation needs to
happen where care happens.

Health innovations rarely fail to commercialize and scale because of the technology alone. They fail because they are developed in isolation from the real-world contexts in which they must ultimately function.

This summit is part of a province-wide Health Everywhere Living Lab initiative designed to connect real-world needs, collaborative design and implementation.

OUR FOUR TRANSITION CHALLENGES

Four themes. Real-world solutions. Lasting impact.
01

Rural & Remote Care

Exploring equitable care pathways for rural and remote communities.

Accessible and equitable physical and mental healthcare for rural communities, including farmers, ranchers and other underrepresented populations.

Summit participants will identify transition barriers and co-design practical approaches grounded in rural realities.
02

Leaving Acute Care

Exploring patient-centred alternative levels of care.

Better pathways for people ready to leave acute hospital care who cannot yet transition safely to their next care setting.

This theme will examine the clinical, community and system connections needed for safer transitions.
03

Life After
Sudden Disability

Exploring life after the sudden onset of disabling conditions.

What spinal cord injury and stroke can teach us about the transition from rehabilitation to living in the community.

The discussion will connect lived experience with rehabilitation, community services and technology-enabled support.
04

Emergency Care &
Unhoused People

Exploring pathways from emergency care for unhoused people.

Creating effective care pathways following discharge from emergency care.

Participants will consider continuity, access, coordination and practical supports after emergency care.
